Fluconazole *
REQUESTING AND COLLECTING

Ordering:
Collect after the 5th dose or at steady state. Collect sample just prior

to next dose (trough sample). If dose changed, remeasure after 5

days.

Several samples throughout dosing interval to estimate AUC (thereby

AUC/MIC ratio) may be reasonable: e.g. sampling at 1, 4, and 24h.

Collection:
Do not use tubes containing separator gel.

available Friday mornings.

Trough: > 2 mg/L for sensitive organisms.

NOTE:

Most evidence suggests the use of AUC/MIC as the parameter most

predictive of fluconazole efficacy. 24h AUC/MIC varies depending on

MIC susceptibility testing methodology: >25 (CLSIa) or >100

(EUCASTb): associated with higher treatment success. - SydPath

currently uses EUCAST method.
