REQUESTING AND COLLECTING

Ordering:
FISH must be ordered on a spearate request form (no other requests

on the form), with relevant clinical history. The test is non-rebatable

from Medicare. If the patient is to be charged a signed agreement to

pay should be included (see request form below). See separate entries

for details on HER-2 NEU and 1p/19q FISH.

Collection:
Either:

- Paraffin embedded Block showing tumour or lymphoma involvement, or



- 2x3-4uM tissue sections on silanised (positively-charged) slides for

each probed requested should be sent to the Cytogenetics/FISH

Laboratory ((02) 8382 9153/9154) for information.

The block will be returned following issue of the final FISH report. The

request MUST be accompanied by the relevant histology report.

SPRO: do not unpack parafin embeded tissue. Deliver immediately to
