Vancomycin
REQUESTING AND COLLECTING

Ordering:
For patients receiving twice-daily intermittent dosing (recommended for

CrCl>60 mL/min), the first trough level is recommended prior to the

fourth dose and should be in the range 15-20 mg/L.

For patients receiving once daily intermittent dosing (recommended for

CrCl 20-60 mL/min) the first trough level is recommended prior to the

third dose and should be in the range 15-20 mg/L.

For patients on continuous infusions, steady state monitoring is

required (usually drawn from the opposite arm to the infusion site) and

the concentration should be between 20-25 mg/L.

In general, the next vancomycin dose should NOT be withheld pending

the vancomycin level result.

In patients with stable renal function, once the therapeutic dose of

vancomycin has been established, twice-weekly ongoing monitoring is

generally adequate.

Collection:
Trough levels (intermittent dosing): Sample just before next dose, do

not withhold dose pending result.

Steady state levels (continuous infusions): Sample after 3-5 half-lives

(approximately 24 h in patients with normal renal function). Do NOT

collect samples from i.v. lines through which vancomycin has been

administered to the patient.

Availability:
Test performed 7 days/24 hours. Results available 2 hours after sample

arrival in laboratory.

Trough: 15-20 mg/L (intermittent dosing), 20-25 mg/L (continuous

infusion)
