Days 8-30 1200 - 1700 ug/L

Days 31-60 1000 - 1500 ug/L

Days 61-90 800 - 1200 ug/L

Days 91-180 700 - 1000 ug/L

Days 181-365 600 - 900 ug/L

Day >365* 600 - 800 ug/L

* if abnormal renal function: 300 - 600 ug/L
